Support Coordination

Support Coordination at Community 3SIX5

At Community 3SIX5, our Support Coordination services are all about empowering you to make the most of your NDIS plan—on your terms.

Our experienced, locally-based Support Coordinators take the time to get to know you, understand your goals, and help you build the skills and confidence to navigate the NDIS with clarity and ease. Whether you're new to the NDIS or just need support getting things moving, we’re here to guide you every step of the way.

🧭 What We Do

Support Coordination is more than just connecting you with services. It’s about helping you:

  • Understand your plan – what your funding covers, how it works, and how to manage it

  • Connect with the right supports – including mainstream, community, and funded services

  • Build your capacity – to manage service providers, make informed decisions, and take control of your supports

  • Strengthen your voice – so you feel confident advocating for your needs and navigating NDIA processes independently

  • Resolve issues – such as service gaps, changes in needs, or conflicts with providers

We focus on creating a support network that truly reflects your needs, values, and lifestyle—so you can live the life you choose, independently and with confidence.

💼 Maintaining Independence and Transparency

We understand the importance of trust and ethical practice. That’s why we have a clear Support Coordination Conflict of Interest Policy in place.
If you choose to access other supports through Community 3SIX5, we’ll ensure your choices are respected, your options are clearly outlined, and your Support Coordinator remains focused on your best interests—independently and without pressure.

💡 We Also Support With:

  • Setting up and managing service agreements and bookings

  • Helping you understand your funding categories and budgets

  • Clarifying what's claimable and what requires NDIA approval or quotes

  • Preparing for and participating in plan reviews

  • Supporting transitions, such as moving to a new home or changing providers

Our approach aligns with the NDIS Practice Standards, supporting your rights, choice, dignity, and inclusion every step of the way.
